Home Health Care Triggers for Respiratory Program:
COPD I Emphysema I Bronchitis / Pneumonia / Pulmonary Hypertension
- Frequent ER visits I hospitalizations
- Advancing chronic pulmonary disease – Increased difficulty with ambulating, balance or gait
- Escalation of symptoms (shortness of breath)
- Oxygen use
- Frequent medication changes
- Declining functional status
Respiratory Program Services:
- Teaching on coping mechanisms for shortness of breath (i.e, pursed-lip and diaphragmatic breathing, anxiety, smoking cessation, and use of incentive spirorneter)
- Teaching on nebulizer treatments
- Energy conservation methods
- Respiratory assessments including 02 saturation
- Teaching on use of 02 and safety related to 02 usage
- Teaching patient / family I caregiver to manage disease
- Assess nutrition, need for small frequent feedings
- Pulmonary Toileting
- Home Health Aide for bathing, dressing, and personal care
- Medication management
