All applicants MUST apply in person. We do NOT accept online applications.
We accept applications at our Boynton Beach office on Tuesday, Wednesday and Thursday between 10:00am and 3:00pm. Professional attire preferred.
Please bring the following with you to fill out application
- Florida Driver’s License
- Social Security Card
- Documents Authorizing Applicant to work in U.S (1 of the following):
- Work Permit
- US Passport
- Certificate of Naturalization
- Birth Certificate
- Voters Registration Card
- Permanent Resident Card
- Physical (Dated within last 6 months and NOT performed by a Chiropractor) Must state applicant is “free of communicable diseases” and be signed by a M.D., AARNP, P.A. or D.O.
- Chest X-Ray / TB Results or PPD
- Level 2 Background Screening
- Copies of Professional Certificates or Licenses
- Initial HIV- 2hrs
- Domestic Violence-2hrs
- Assisting with Self Medication- 2hr certificate
- Alzheimer’s- 2hrs
- Infection Control
- Proof of Automobile Insurance
- Proof of Automobile Registration
- Professional Liability Insurance ( Can be done during application process)
- Two professional / work references with fax / phone numbers
