Neurogenic Bladder / UTI/ Urostomy / Dialysis
- Complex therapies /dialysis/ renal transplants f multiple organ failure
- Frequent ER visits hospitalizations
- Escalation of symptoms (shortness of breath, nausea, nutritional deficits)
- Frequent medication changes
- Declining functional status
- Declining nutritional status
- Advanced renal disease
- IV therapy (Antibiotics, Anti-Virals, Anti-Furigals, Post-Transplant Medication, Immune Therapy)
Renal / Urology Program Services:
- Fluid assessment and education on increased fluid needs
- RN available to manage medications and coordinate care with primary care physician and specialists
- Comfort / Pain assessment and pain medication management
- Assess urinary elimination
- Skin assessment
- Teach Patient / Family f Caregiver to manage disease
- Catheter teaching, care and monitoring
- Home Health Aide available to assist patients with bathing, dressing, and grooming if needed
