Triggers for Cardiac Program Eligibility:

  • Frequent ER visits hospitalizations
  • Escalation of symptoms (chest pain, shortness of breath, dizziness)
  • Frequent medication changes • Oxygen use
  • Inotropic administration
  • Declining functional status
  • Depression Anxiety / Suffering
  • NYHA (New ‘for* Heart Association) Class 1-4

Cardiac Program Services:

  • Cardio-Respiratory assessment
  • Assessment of CHF including edema and dyspnea
  • Medication management by RN
  • Teaching and use of Cl2 safety and PRN 02 saturation
  • Teaching and monitoring of gradual exercise program including target heart rate
  • Nutrition assessment and teaching on low sodium diet and fluid restrictions
  • Teaching on necessity for daily weights, recording, and reporting increases to physician
  • Energy conservation techniques
