Triggers for Cardiac Program Eligibility:
- Frequent ER visits hospitalizations
- Escalation of symptoms (chest pain, shortness of breath, dizziness)
- Frequent medication changes • Oxygen use
- Inotropic administration
- Declining functional status
- Depression Anxiety / Suffering
- NYHA (New ‘for* Heart Association) Class 1-4
Cardiac Program Services:
- Cardio-Respiratory assessment
- Assessment of CHF including edema and dyspnea
- Medication management by RN
- Teaching and use of Cl2 safety and PRN 02 saturation
- Teaching and monitoring of gradual exercise program including target heart rate
- Nutrition assessment and teaching on low sodium diet and fluid restrictions
- Teaching on necessity for daily weights, recording, and reporting increases to physician
- Energy conservation techniques
