- Teach use of glucometer and need to check blood sugars
- Medication management a Nutritional assessment and teaching
- Skin assessment for actual or potential impairment
- Woundcare and monitoring of wounds to reduce risk of infection
- Comfort /pain assessment, pain medication management
- Safety /exercise program and energy conservation
- Home Health Aide for bathing, dressing, and personal care
- Assess cognitive/ sensory and perceptual impairment
