- Frequent ER visits / hospitalizations

- Newly diagnosed or advanced stages of cancer
- Escalation of symptoms
- Complications from Chemotheapy or Radiation Therapy require support

- Declining nutritional status
- Declining functional status
- Anxiety Depression Coping Issues
Cancer Care Program Services:
- Assess patients knowledge of disease process and current treatments
- Teaching on complications of disease and preventions
- Physical Therapist available to develop home exercise program
- Certified Lymphadema Therapist
- Comfort /Pain assessments and pain medication management
- Teaching on side effect of antineoplastic radiation therapy
- Nutritional Assessment- Including PEG Tube feedings and Teaching
- Administration and teaching of Home Infusion Therapy ( IV Antibiotics, IV Fluids, etc…)
- Assess support systems and community resources available for additional support
